Apartment Information

Please read and complete all parts of this application that apply to you.  Upon completion, return to the office for processing along with the listed documents:

▪  Birth Certificate for each family member

▪  Social Security Cards for each family member

▪  Marriage Certificate if apply

▪  Companion or roommate is required to submit an application also

▪  Criminal Report for family member age 17 and older

▪  Copies of photo identification or drivers license of family member age 17 and older

▪  Copies of photo identification of family member age 10 and older

   COPY OF CURRENT REPORTS

▪  Proof of current income/4 previous check stubs for family members 18 years or older.

   (letter provided by your employer is acceptable.)

▪  Income tax return for 2 years/business owner. (Profit and Loss Statement)

▪  Affidavit/notarized statement of income

▪  Copy of voter registration card. (if available)
